For your formation, it depends who you have on the pitch (click the yellow arrow next to the player to substitute them with a player on the bench). e.g. If you have more strikers it might be 4-3-3 rather than 4-4-2.

To change your captain, there should be a little pop-down bar for you to select the captain.

I don't know about that, the likes of Gattuso and Makelele aren't as affective in fantasy football as they are on the pitch.
